Critical illness insurance
A serious diagnosis is not the same thing as a payable claim.
Compare 11 sourced cover records from 10 insurers. Ask what the policy pays for the same clinical event, how long you must survive it, and what benefit remains afterward.

Start with the money
Which cost would the payout absorb?
Use one of these purposes to frame a comparison. A policy will not guarantee every diagnosis; its definition, severity tier and claim conditions still apply.
Step 2 of 3 · Policy shortlist
Compare 11 critical illness policies
11 sourced policies from 10 legal insurers; each record names its carrier and official evidence. Insurers linked to this shortlist are shown for factual comparison, not endorsement.
Two 1Life designs count as one insurer. Some plans need a life policy, professional membership or store-card access; check that before price.
All prices need an individual quote; published examples are not comparable entry premiums.
Age, medical history, smoker status, sum insured, benefit design and whether a life plan is required change the price. Confirm the current premium, policy wording, limits, excesses, waiting periods and underwriting outcome with the provider.
Rating (1-5) counts five source checks: legal identity, product scope, entry conditions, a comparable price basis and claims instructions. A tick confirms only the named field in the source; an unconfirmed check is not a missing policy benefit. Scores are not customer reviews, claims performance or recommendations. Each record has a review date and owner. Our comparison methodology.

A source-check tick confirms only that the named information was found in the reviewed official material. An unconfirmed field does not prove the policy lacks a benefit. Compare the accepted wording, schedule and individual quote before deciding.
The clinical contract
A diagnosis name is too blunt a comparison
“Cancer”, “heart attack” and “stroke” can each have multiple clinical definitions and severity levels. One contract may pay part of the sum for an early stage, another may reserve payment for a higher threshold. Ask for the precise pathology, procedure or specialist evidence in the current wording. The severe-illness cover guide shows how to read these definitions.
Find the event
Use the diagnosis or procedure in the policy schedule, not a marketing condition count.
Find the percentage
Apply the selected benefit's severity tier to the insured sum. A 25% event on R400,000 illustrates R100,000, not the full R400,000.
Find what remains
Check related-condition, repeat-claim and accelerated-life rules after any partial payout.
To understand the surrounding cover, compare life insurance, monthly income protection, medical scheme and health cover and gap cover as separate jobs.
Benefit design
Do not compare unequal payout promises
| Design | What can pay | Watch before choosing |
|---|---|---|
| Tiered severe illness | A percentage follows the defined severity | An early-stage event may pay less than the headline amount |
| Core-condition 100% option | Selected major events may pay the full amount at a qualifying threshold | Other events may still be tiered or excluded |
| Specialist or restricted-condition plan | A narrower diagnosis set, sometimes including children | Do not compare its condition count with a broad policy |
| Accelerated life benefit | Illness cash can reduce the remaining death benefit | Check whether the life and illness sums are linked |
| Additional life-plan benefit | A separately selected illness sum may sit alongside death cover | Ask which premium and later claim rules apply to both benefits |
The illustration above is a calculation example, not a claim prediction. The issued policy can cap partial benefits or reduce an accelerated death benefit.
Quote discipline
Five questions to put beside every price
| Policy test | Why it matters | Ask the insurer |
|---|---|---|
| Clinical definition | Everyday diagnosis labels are too broad | Which pathology, procedure, impairment or specialist evidence triggers this benefit? |
| Survival period | Some contracts require survival after diagnosis | What period, if any, applies to this event? |
| First and later claims | A partial claim may reduce or preserve later cover | How much remains, and can a related condition pay again? |
| Premium pattern | A cheap first year may not remain cheap | What can increase, and when? |
| Policy access | Some benefits attach to a life plan | Must I hold another policy, and is this accelerated or additional? |
People with an earlier diagnosis should read how underwriting treats a health history and how to prepare disclosures. Neither guide promises acceptance.
What an accepted benefit can do
- A listed diagnosis or procedure that satisfies the wording and severity test
- A partial or full percentage of the sum insured, where the selected benefit provides it
- Some repeat or unrelated conditions if the contract explicitly allows further claims
Where assumptions fail
- A diagnosis that falls below the policy's clinical threshold
- An excluded pre-existing condition or event before cover starts
- A later claim after the relevant benefit has been exhausted
- A payment assumed to replace medical-scheme, gap-cover or income benefits
From recovery budget to accepted wording
Name the cash need
List recovery leave, debt, transport, support at home and other costs not already funded.
Compare a real scenario
Use the same proposed sum insured and a defined cancer, heart attack or stroke scenario across wordings.
Read the payout table
Record full, partial, repeat and survival rules, not just the number of conditions.
Disclose medical history
Answer each underwriting question accurately and retain the final accepted exclusions.
Check continuation
Confirm whether life or illness cover reduces after a claim and how premiums change.
If a monthly earnings gap is the main worry, use the lump sum versus income guide before buying a second capital policy.
Claims flow
A claim is measured against the wording
Keep the final policy document, not just an older brochure. Ask which event and severity category the insurer used and what remains after any payment.
Notify the insurer
Use the issued policy's claim route and record the date and reference.
Gather medical proof
Obtain the named specialist report, diagnosis, pathology or procedure evidence requested.
Check the definition
Ask which wording version and severity tier the assessor applied.
Review the decision
Confirm the benefit percentage, any survival condition and remaining cover; escalate a dispute through the insurer and NFO if needed.
Documents worth keeping ready
- Issued policy and benefit schedule
- Application and disclosed health history
- Specialist reports and test results
- Claims form and insurer correspondence
- Written decision and remaining-benefit statement
Balanced view
- Flexible cash for costs beyond treatment invoices
- May support recovery even if the person remains employed
- Some policies allow partial or multiple claims
Limits
- The clinical wording, not the illness name, decides payment
- Survival and pre-existing-condition rules can change eligibility
- A rider may require and affect another policy

Questions answered
Frequently asked questions
Is dread disease the same as critical illness insurance?
The terms are commonly used for the same class of defined-event cover. Compare the actual insured conditions and policy wording, not the name.
Does an early cancer diagnosis pay the full sum insured?
Not automatically. Stage, pathology and the selected severity design determine whether the event qualifies and at what percentage.
Can I buy it without life cover?
Some South African options are standalone; others are benefits on a life plan. Verify the buying route and whether an illness claim reduces the death benefit.
Do I need it if I have medical aid?
Medical schemes fund eligible healthcare under scheme rules. Critical illness insurance pays cash for a qualifying policy event; neither substitutes for the other.
Is there a standard waiting or survival period?
No single period applies to every insurer and event. Read the accepted contract for condition-specific waiting and survival rules.
Can I compare prices here?
The records are a source-backed shortlist, not personal quotes. Request the same sum insured, age, health and benefit design before comparing premiums.
